Facet Joint Health, Low Back Pain and Upper Back Pain

Simply put, the facet joint links the vertebrae of the human spinal column and allow for the articulation of the spine. When they facet joints are not functioning properly, or when they become degenerated, develop arthritis, or slip, they may be the cause of low back pain and upper back pain.  In addition to providing mobility, the facet joints also help stabilize the spinal column by being oriented in such a way that they decrease the chances of preventing forward vertebral slippage (spondylolisthesis). The ends of the facet joints are covered in cartilage, which allows for pain-free operation over a normal range of motion (bending, twisting, lifting, sitting, standing, etc.). While the cartilage does break down naturally over time (in this manner resembling cartilage in other parts of the body), the facet joints are still quite durable if direct physical trauma (such as from an accident) is not an issue. The condition known as spondylolisthesis exists when one vertebral body is slipped forward upon the vertebral body below.  This is also known as anterolisthesis.  When the vertebral body is slipped backward upon the vertebral body below, the condition is known as retrolisthesis.  Spondylolisthesis may occur as a result of degenerative changes of the facet joint.  When a patient has a condition known as spondylolysis, the patient is prone to develop spondylolisthesis.  Spondylolysis is a condition where there is a gap in what may be considered the roof over the spinal canal.  It may occur from an injury, or a person may be born with it.  When this condition occurs, the facet joint is no longer able to help support the vertebral bodies from slipping forward upon each other, and instead, all of this burden is placed upon the intervertebral disc, which separates two adjacent vertebral bodies.  Spondylolisthesis, whether anterolisthesis or retrolisthesis, may be related to low back pain or upper back pain, as it relates to possible instability of the spinal column.

With all of these essential functions, the facet joints are a little-known but important means to preventing low back pain and upper back pain. These joints, along with spinal discs (the shock-absorbing pads between our vertebrae) help relieve pressure on the spinal nerves during routine motion, lowering the incidence of nerve compression (and accompanying sharp low back pain). Over time, however, the facet joints begin to break down just like any other mechanical component in the human body. Repetitive bending, lifting, and twisting can all accelerate the natural degenerative processes in the human spinal column, and the facet joints are no exception. The cartilage on the ends of the joints breaks down as spinal discs become thinner, harder, and less capable of bearing shock as we grow older. All of this leads to higher incidence of compressed (pinched) spinal nerves, which in turn can cause muscle weakness, numbness, low back pain, upper back pain, and decreases in overall mobility.

As such, maintaining the health of our facet joints as we age should be a priority for everyone. Patients with chronic low back pain and upper back pain are advised to explore the option of physical therapy or chiropractic designed specifically for the spine. Typically, spine-specific physical therapy programs use a regimen of core-strengthening exercises to build muscles along the spinal column and throughout the torso. Stronger core muscles are more capable of relieving load-bearing stress on other degenerated portions of the spine, such as the discs and facet joints. Though the human body may be an incredible machine, its mechanical components do not escape wear and tear. Conventional back surgery is a costly process with a long period of post-operative care. Depending on the severity of the damage to the spinal column, months of physical therapy may be necessary to re-develop damaged muscles and restore patient mobility. Physicians generally make deep lacerations into the muscles along the spinal column, prolonging the recovery process for the sake of having vision of the surgical site. Minimally-invasive back surgery has improved upon traditional techniques by allowing for smaller, less destructive incisions at the surgical site. Rather than deep lacerations which are then pulled aside allowing the surgeon to see the injury, minimally-invasive back surgery uses a tubular retractor to open a direct pipeline to the site. Pieces necessary for the surgery, such as screws or plates to stabilize a compromised spinal column, are then fed through the tubular retractor to complete the surgery.

Robot-guided back surgery looks to further improve this process. Essentially, robot-guided back surgery uses a system that compiles data from a computerized tomography scan (CT scan) of each patient. This scan is then fed into a 3D planning system which allows the surgeon to make calculations in three-dimensional space prior to the surgery beginning. A robot also accompanies the surgeon during the operation, mainly to reduce the possibility of human error by indicating with a high degree of precision the places where the surgeon needs to make his or her adjustments. Ideally, robot-guided back surgery will serve to reduce the post-operative care period even further, necessitating less intensive physical therapy to regain patient mobility.

The laser is sometimes used in surgery.  The laser is a tool which allows the surgeon to vaporize tissue, without the need for pulling or cutting it.  The laser can cut, or coagulate, depending upon the type of laser, and the settings of the laser.

Typically, there are nine discrete segments of the sacral vertebrae in early development. Five of these segments usually form the sacrum and sacral vertebrae, while the other four become the coccyx. Essentially, the orientation of the sacrum and its corresponding sacral vertebrae are partially responsible for how humans walk. If the sacrum were turned at a different orientation, bipedal motion would not be possible. Even a slight change to the orientation of the sacrum and corresponding sacroiliac joints would result in a vastly different walking motion.
Due to their orientation in articulation with the ilium (hip bone), the sacral vertebrae are susceptible to sacroiliac joint dysfunction (SJD). In SJD patients, pain is usually more prominent on one side of the lower back, though it may radiate down one leg or the other. Though the precise cause of sacroiliac joint dysfunction may be difficult to pinpoint, typically SJD is caused by either 1) overworking the sacroiliac joints, such as in labor or hard exercise or 2) too little physical activity. These problems (hypermobility and hypomobility, respectively) can also cause problems in other parts of the spinal column and other regions of the body. Hypermobility tends to degrade the sacroiliac joints through increased wear, load-bearing, and stress, while hypomobility can deny the joints nutrition by reducing regular access to oxygen.
Care for the sacroiliac joints echoes care for the rest of the spinal column and the body in general—regular exercise, rest, healthy dieting, and weight management. Maintaining a healthy body weight can reduce the chances of sacroiliac joint dysfunction, while regular exercise helps expose sacroiliac joints to increased levels of oxygen and nutrition.

The lumbar vertebrae are the lowest five bones of the spine, designated as L1-L5. These bones are significantly larger than other vertebrae in the spinal column, as well as differing substantially in shape. The lowest part of the spinal column is also responsible for bearing much of the body’s weight in the sitting and standing positions. This means that the intervertebral disks—pads in between each vertebra that help the body absorb the shock of repetitive movement—are the most prone to injury in the lumbar spine. Though the lower part of the spinal column is designed for both power and flexibility, there are sundry ways in which its function can become impaired.
Many of these lumbar spine-related problems are the result of injury, such as repetitive load-bearing stress sustained during manual labor. Other common causes include excessive weight, which can in turn cause herniated disks in the spinal column. When evaluating lower back pain, patients are advised to monitor closely any pain that seems to travel or “shoot” down the legs or arms. This can be a sign of compressed spinal nerves, caused when the disk provides insufficient protection against the stress and the vertebrae press down upon the spinal nerves. Due to the lumbar vertebrae’s location and role in load-bearing activities, an injury can be especially painful and debilitating. Patients are advised to seek medical consultation in order to better evaluate possible courses of action.
Should the integrity of the lumbar spinal column be compromised, physical rehabilitation will often form an essential component of a patient’s recovery. Physical therapy will include core-strengthening exercises to boost the spine’s support network of muscles, ligaments, and tendons. A stronger core translates into less load-bearing for the lumbar spine, which can reduce the possibility of future injury as well as aid in reducing a patient’s back pain. More serious injuries—such as severe trauma or structural deformities—may necessitate spinal surgery. If physical therapy, core strengthening, and weight management have proven ineffective, spinal specialists can help in determining what surgical procedures may alleviate the problem.
